How Pleasant Pediatrics handles information collected through our website.

Information you provide to us as a patient through our patient portal, in our offices, or through written/electronic forms used for medical care is covered by our separate HIPAA Notice of Privacy Practices.

Information you provide. When you fill out a form on the Site (contact form, feedback form, appointment booking, intake forms, etc.), we may collect your name, date of birth, address, phone number, email address, insurance information, and any other information you choose to submit.

Information collected automatically. When you visit the Site, our servers and third-party services may automatically collect:

  • IP address and approximate geographic location
  • Browser type, operating system, and device identifiers
  • Pages you visit, the date and time of your visit, and the page you came from (referrer)
  • Interactions with the Site, such as clicks and scrolls
  • Cookies and similar technologies (small data files stored on your device)

2. Cookies and Tracking Technologies

We use cookies and similar technologies to operate the Site and, with your consent, to understand how visitors use it.   • Service providers that help us operate the Site, such as hosting providers, MongoDB (temporary form storage), Twilio (SMS and chat), email delivery, payment processors, and Google (Analytics, Tag Manager, Ads).
  • Our electronic health record system (eClinicalWorks) when you submit forms or book appointments.
  • Legal and regulatory authorities when required by law or to protect rights and safety.

Some of these third parties may collect information directly from your device (for example, IP address) when their scripts are loaded on the Site. Their use of that information is governed by their own privacy policies.

5. Data Retention

Form submissions stored temporarily in MongoDB are automatically deleted after 30 days. Information transferred into our medical record system is retained in accordance with applicable medical recordkeeping laws. Analytics data retention is governed by the third-party provider (e.g., Google).

6. Your Choices and Rights

  • Cookies and tracking: Use the cookie banner or link to accept or decline non-essential cookies.
  • Do Not Track: Our Site does not respond to browser Do Not Track signals at this time, but no tracking occurs unless you accept on the banner.
  • California residents (CCPA / CIPA): You have the right to know what personal information we collect about you, to request deletion of that information, to opt out of the “sale” or “sharing” of personal information (we do not sell personal information), and to be free from discrimination for exercising these rights. We use industry-standard administrative, technical, and physical safeguards to protect information collected through the Site, including encryption in transit, rate limiting, and access controls. No method of transmission over the Internet is 100% secure, however, and we cannot guarantee absolute security.

9. Children’s Privacy

Our practice serves patients through age 21. The Site is intended for use by parents and guardians. We do not knowingly collect information directly from children under 13 without parental involvement. If you believe a child has provided information without parental consent, please contact us so we can delete it.

10. Changes to This Policy

We may update this policy from time to time. When we do, we will change the “Effective Date” above. Material changes will be communicated through the Site.
